Parent or guardian agreement

PARTICIPATE.
UNDERSTAND THE RISKS.

Participation agreement, acknowledgment of athletic risks, limited release, and emergency-care permission for programs operated by Future Ballers, LLC ("Future Ballers"). Please read the entire agreement before accepting it.

1. Who is agreeing and what is covered

I am at least 18 years old and am either the athlete signing for myself or the athlete's parent or legal guardian with authority to give participation and emergency-care permission. The athlete and the session or program covered are identified in the booking record. I will not sign for another person's child without lawful authority.

This agreement covers basketball and volleyball training, classes, camps, clinics, and supervised gym activity provided by Future Ballers, LLC at 2162 County Road 761, Brookland, Arkansas, or another location shown for the activity. It does not cover unrelated activities, transportation, overnight care, or a birthday-party rental, which has its own terms.

2. Athletic risks and voluntary participation

Basketball, volleyball, conditioning, and gym activities involve movement, jumping, landing, falls, contact with balls or equipment, collisions with people, fatigue, and overexertion. Injuries can include bruises, sprains, fractures, concussion, serious head or spinal injury, permanent disability, or death. Illness and other risks may also occur. Instruction, supervision, and safety equipment cannot eliminate every risk.

I understand these risks and voluntarily choose participation for myself or give permission for the identified child to participate. An athlete may stop an activity, report discomfort or an injury, and ask for help at any time. No improvement, scholarship, team placement, or athletic outcome is guaranteed.

3. Health, restrictions, and safety

I will provide accurate emergency-contact information and tell Future Ballers about any restrictions or accommodations relevant to safe participation. I will seek appropriate medical advice when I am unsure whether an activity is suitable. This is not a certification of perfect health or permission for staff to diagnose or treat a condition.

Please do not put medical details in booking notes or ordinary email; contact Future Ballers to share a safety need privately. Athletes must report injuries or concerning symptoms promptly, follow safety instructions, wear appropriate court shoes, and stop when directed. Staff may pause participation for a safety concern. See the concussion and return-to-play information.

4. Emergency assistance

If an emergency occurs, I authorize staff to call emergency services, provide first aid within their training, and seek evaluation or care reasonably needed for the athlete. Staff will try to contact the guardian or designated emergency contact without delaying urgently needed help. This permission is limited to emergency assistance; it does not authorize elective treatment or replace a medical provider's own consent requirements.

A medical professional may not be on site. Responsibility for medical bills and insurance coverage is determined by applicable law and the relevant coverage; this agreement does not promise that Future Ballers will pay all expenses or excuse any responsibility it otherwise has.

5. Limited release — important limits

To the extent Arkansas law allows, I release only claims that I personally own against Future Ballers, LLC and its owners, employees, and assigned coaches for injury or loss caused solely by the inherent athletic risks described in this agreement.

This limited release does not release negligence, gross negligence, reckless or intentional misconduct, abuse, or a breach of a duty that cannot lawfully be waived. It does not release a child's independent claims, require a guardian to indemnify Future Ballers for a child's claims, or waive rights belonging to anyone else. No arbitration, jury-trial waiver, or obligation to pay Future Ballers' legal fees is imposed.

Nothing in the risk acknowledgment excuses unsafe conduct. If any provision cannot be enforced, the remaining provisions continue to apply to the extent the law permits.

6. Supervision and separate choices

I will follow the booking and facility policies, keep contact details current, and arrange timely pickup. Staff may stop unsafe or disruptive conduct. Participation permission is separate from the booking's payment and cancellation terms.

Photo or video permission is optional, starts unselected, and is described in a separate document. Declining it does not affect training eligibility. This agreement does not sign me up for marketing messages or authorize Future Ballers to store payment cards.

7. Electronic acceptance and your copy

I may accept this agreement electronically for the identified athlete and activity by actively checking the required box and confirming my authority. Opening this page, signing in, or making a payment alone is not acceptance.

I may print or save this agreement before accepting, and I can contact Future Ballers to request a copy or ask about a paper alternative. Future Ballers keeps the exact wording and version I accepted, the account and athlete it applies to, and the date and time. A later update does not replace the version I already accepted for an existing booking.

8. Questions

Contact futureballers2026@gmail.com before accepting if anything is unclear. Do not email passwords, card information, or detailed medical records.
